This Questionnaire is for all those who have bought a book or DVD recommended in the LIFE: What’s it all about? booklet, from either of the bookshops named in it, or who borrow one from Heswall Library. There is no obligation to complete it, but it will be very helpful to ACEP in deciding what we might do next if you are willing to do so. There are only 10 Easy to Answer Questions, which shouldn't take more than a minute or two.
When you phone, if you start your conversation by saying you have received a copy of the ‘Life’ booklet, the person answering will know at once what you are phoning about.
